QR Codes and Google Forms:
Interested in replicating the QR code activity?

For help creating a Google form in order to collect responses, click here.
To create your own QR code, you have some options:
  1. Go to http://goo.gl and log into your Google account. Copy and paste the web address from the "live" Google form, and goo.gl will provide a short-cut link and a QR code to share.
  2. Go to http//qrcode.kayway.com. Copy and paste the web address from "live" Google form, and change the format to "Static." Click on Generate and receive a QR code to share.
  3. If using Google Chrome as your web browser, use the menu button on the far right to access Tools --> Extensions OR in Google Chrome type chrome:extensions// in the address bar. In the Extensions Store, search for "goo.gl URL Shortener"and download the "app" to your computer--this extension/"app" will be added to your tool bar. When you visit any website, click on the new goo.gl icon on your toolbar and receive an instant website short cut and a QR code.
